Our case manager does an initial intake meeting with our guests to assess their needs and pinpoint the obstacles that are preventing them from obtaining permanent housing. The case manager then creates a plan with specific goals to help the family meet their needs and overcome the barriers. The case manager meets with each family weekly to assess their progress. The case manager also makes sure that the children are up-to-date on their immunizations and that they are registered for school. The case manager is responsible for maintaining contact with the host coordinators about the guests. This may include work schedules, absences from dinner, or special dietary needs. Our case manager will also conduct follow-up (aftercare) sessions with guests who have graduated from the program.
Also our case manager meets on a monthly basis with the guests who have graduated from the program and are now a part of the Rapid Rehousing program. Clients who meet the requirements for the program and who are deemed eligible for participation in the program are provided with rental assistance and case management services. The case manager helps these participants to create a plan which will help to enable them to sustain their homes and lives while they are in the Rapid Rehousing program as well as after they have completed the program
